Form of Declaration for Surviving
Officer or Soldier.
State of Arkansas,
County of Pike,
On this the 22nd day of May A.D. 1852, personally appeared before me,
Jackson Brock, a Justice of the Peace duly authorized by law to administer
oaths within and for the County and State above mentioned, David Mobley, a
resident of the County and State aforesaid, who being duly sworn according
to law declares: that he is the identical David Mobley who was attached to
Captain Thomas Wells company in a Regiment commanded by Col. Hammonds in the
State of Tennessee in the War of 1812, as set forth in his declaration on
file and that (his) certificate of discharge which he received is lost or
mislaid, so that the same is not now nor was it at the time of filing his
declaration, in his possession.

I, Thomas K. Dossey, Clerk of the Circuit Court in and for the County and
State aforesaid, do hereby certify: that Jackson Brock, whose genuine
signature appears above is and was at the time of signing the same, a
Justice of the Peace in and for the County and State aforesaid, duly
commissioned and sworn and that all his official acts as such are entitled
to full faith and credit, and that the foregoing Circuit Court is a court of
record having general jurisdiction.
In testimony whereof, I have hereunto set my hand and affixed the seal of
said Court at Murfreesboro' this 22nd May 1852.
